Hereby l, the applicant, give my agent and third parties (Operators) whom my agent cooperates with, permission to process my personal information as prescribed by the Protection of Personal information Act (PoPlA, 2013) for the following purposes:  

1. Handling of labor matters and admin related thereto, e.g. contracts, application forms etc.
2. Submission of application forms to various prospective employers in America etc. 
3. Information of my next of kin in case of any emergency where we might need to contact them on your behalf.
4. Providing information when it is lawfully requested by a relevant government institution, e.g. The Department of Labor, SARS or SA Police Service. 

My agent undertakes to use only the minimum, relevant information about me as is necessary, and that my personal information will always be kept safe and correct and will not be revealed to any unauthorized parties.  

The recruitment agent will maintain reasonable security measures as required by Section 19 of the POPIA Act regarding any of my personal information that comes into his possession in consequence of his rights and obligations to me, the applicant.  

Any breach of the integrity of my personal information will be communicated to me and to the Regulator of the POPIA Act as soon as possible.
